User Guide: Training Request Form
Overview
This Training Request Form allows employees to formally request training opportunities. The form collects all necessary information for managers and HR to evaluate and approve training requests efficiently.
How to Use the Form
1. Employee Information Section
- Full Name: Enter your complete legal name
- Employee ID: Provide your company identification number
- Department: Specify which department you work in
- Job Title: Enter your current position
- Email: Provide your work email address
- Phone Number: Enter your contact number
- Manager's Name: Provide your direct supervisor's full name
- Manager's Email: Enter your manager's email address
2. Training Information Section
- Training Course/Program Title: Specify the exact name of the training
- Training Provider: Name the organization offering the training
- Training Format: Select the delivery method from the dropdown
- Training Category: Choose the most relevant category for the training
- Start Date & End Date: Specify the training duration
- Duration: Enter the total hours of training
- Estimated Cost: Provide the total cost including fees, materials, travel, etc.
- Training Location: Specify where the training will occur (if applicable)
- Training Objectives: Explain what you hope to learn and how it will benefit your work
- Business Justification: Describe how this training will help achieve business goals
3. Additional Information Section
- Certification: Indicate if the training results in a certification
- Prerequisites: List any requirements needed before taking the training
- Additional Notes: Include any other relevant information
4. Approval Section
- Applicant Signature: Type your full name as a digital signature
- Date: Enter the current date
- Manager's Signature: Your manager will complete this field
- HR Signature: HR will complete this field upon approval
Submission Process
- Complete all required fields (marked with *)
- Click the "Print Form" button
- Review the printed document
- Submit the printed form to your manager
- Your manager will review and forward to HR if approved
- HR will make the final decision and notify you
Tips for Completion
- Provide detailed business justifications to increase approval chances
- Research training costs thoroughly to provide accurate estimates
- Consult with your manager before submitting the form
- Submit requests well in advance of training start dates
Troubleshooting
- If fields don't validate, check that all required fields are completed
- For date fields, use the calendar picker or format as YYYY-MM-DD
- If the print button doesn't work, use your browser's print function (Ctrl+P or Cmd+P)
