User Guide: Diversity Survey Form
Overview
This Diversity Survey Form is designed to help organizations collect demographic information from employees or members to better understand and improve diversity and inclusion initiatives. The form is web-based, user-friendly, and can be printed for physical submissions.
How to Use the Form
1. Accessing the Form
- Open the form in any modern web browser (Chrome, Firefox, Safari, Edge)
- The form will load with all sections visible
2. Completing the Form
- Fill out each section of the form as appropriate
- Fields marked with a red asterisk (<span class="required">*</span>) are required
- Use the tab key or click to navigate between fields
3. Form Sections
Personal Information
- Provide your full name (required), email, department, position, and date
- This information helps identify respondents while maintaining confidentiality
Demographic Information
- Select your gender identity, age range, ethnicity/race, and country of origin
- All questions include a "Prefer not to say" option
Disability Status
- Indicate if you identify as having a disability
- Optionally specify the type of disability
Education Background
- Select your highest level of education
- Indicate if you're a first-generation college graduate
Additional Information
- Provide information about veteran status, LGBTQ+ identification
- Optionally share religious affiliation and languages spoken
Optional Feedback
- Share suggestions for improving diversity and inclusion
- Add any additional comments you'd like to include
4. Printing the Form
- Click the "Print Form" button at the bottom of the page
- The browser's print dialog will appear
- Adjust print settings as needed and print the form
- When printed, the button will not appear on the hard copy
5. Submission Process
- After completing the form, print it for physical submission
- Alternatively, you can save the form as a PDF (using your browser's print to PDF feature) for digital submission
- Submit according to your organization's specified process

- All information provided is confidential
- Data will be used only for aggregate reporting and diversity initiatives
- Individual responses will not be shared without explicit permission
