How to Complete the Form
Personal Information Section
- First Name & Last Name: Enter your legal first and last names exactly as they appear on your government-issued identification.
- Social Security Number (SSN): Provide your 9-digit SSN without hyphens.
- Date of Birth: Use the calendar picker or enter in MM/DD/YYYY format.
- Address: Enter your complete street address, city, state, and ZIP code.
- Filing Status: Select the option that best describes your current filing status:
- Single: Unmarried or legally separated
- Married Filing Jointly: Married couples combining income
- Married Filing Separately: Married couples filing separately
- Head of Household: Unmarried with qualifying dependents
- Qualifying Widow(er): Spouse died within past two years with dependent child
Income Information Section
- Wages, Salaries, Tips: Enter the total from your W-2 forms (box 1).
- Taxable Interest Income: Report interest from bank accounts, CDs, etc. (Form 1099-INT).
- Ordinary Dividends: Report dividend income (Form 1099-DIV).
- Business Income: Report net profit or loss from business (Schedule C).
- Capital Gains: Report profits from sale of assets like stocks or property.
- IRA Distributions: Report withdrawals from retirement accounts.
- Other Income: Describe and amount of any other taxable income.
Deductions & Credits Section
- IRA Contributions: Report contributions to traditional IRAs.
- Student Loan Interest: Report interest paid on qualified student loans.
- Tuition and Fees: Report qualified education expenses.
- Medical Expenses: Report expenses that exceed 7.5% of your adjusted gross income.
- Deduction Type: Select either Standard Deduction (fixed amount based on filing status) or Itemized Deduction (if your qualifying expenses exceed the standard amount).
Tax Payments Section
- Federal Income Tax Withheld: Total federal tax withheld from your W-2s and 1099s.
- Estimated Tax Payments: Total quarterly estimated tax payments made.
- Earned Income Credit: If eligible, claim this refundable tax credit.
- Additional Credits: Report any other tax credits you're claiming.
Signature Section
- Signature: Provide your legal signature.
- Date: Enter the current date.
- Contact Information: Provide your daytime phone number and email address for any follow-up questions.
Printing Instructions
- Click the "Print Form" button at the top of the form
- Review the print preview to ensure all information is visible
