This user guide will help you understand how to properly complete the Fund Transfer Request Form.
Overview
The Fund Transfer Request Form is used to request transfers between bank accounts. It collects all necessary information to process domestic and international transfers securely.
Form Sections
1. Sender Information
- Full Name: Your complete legal name as it appears on your bank account
- Account Number: Your bank account number from which funds will be debited
- Address: Your complete mailing address (street, city, state, ZIP code, country)
- Phone Number: A contact number where you can be reached if needed
2. Recipient Information
- Full Name: Complete legal name of the recipient as it appears on their bank account
- Account Number: Recipient's bank account number
- Bank Name: The name of the recipient's financial institution
- Bank Address: Physical address of the recipient's bank (optional but recommended)
- Routing Number: ABA number (for US banks) or SWIFT/BIC code (for international transfers)
- IBAN: International Bank Account Number (required for transfers to many countries)
3. Transfer Details
- Amount: The exact amount to be transferred
- Currency: Select the appropriate currency for the transfer
- Transfer Date: When you want the transfer to be processed
- Transfer Type:
- Domestic: Within the same country
- International: Between different countries
- ACH: Automated Clearing House (electronic network for financial transactions)
- Same Bank: Between accounts at the same financial institution
- Reason for Transfer: Brief description of the transfer purpose
- Additional Instructions: Any special handling instructions
4. Fees and Authorization
- Fee Responsibility: Who will bear the transfer fees
- Authorization: Must be checked to confirm you authorize the transaction
- Signature: Your handwritten signature (on the printed form)
- Date: Date of signing the form
How to Complete the Form
Digital Completion:
- Fill in all fields marked with an asterisk (*) as they are required
- Provide accurate information matching your bank records
- Select the appropriate options from dropdown menus
- Click the "Print Form" button when finished
After Printing:
- Review all information for accuracy
- Sign and date the form in the designated area
- Submit the completed form to your bank representative
