Fitness Assessment Form - User Guide
This user guide will help you understand how to properly use the Fitness Assessment Form to record client information and fitness metrics.
Overview
The Fitness Assessment Form is designed for fitness professionals to:
- Collect client personal and health information
- Record body measurements and fitness test results
- Document observations and recommendations
- Create a printable record for client files
Form Sections
1. Personal Information
- Full Name: Enter the client's complete name
- Date: The date of the assessment (auto-filled to today's date by default)
- Date of Birth: Client's birthdate
- Gender: Select from the dropdown options
- Contact Information: Phone, email, and physical address
- Emergency Contact: Name and phone number of emergency contact
2. Health History
- Medical Conditions: Check all that apply from the listed options
- Medications: List any current medications
- Previous Injuries: Document any past injuries that might affect training
- Allergies: Note any allergies (food, medication, environmental)
- Activity Level: Select the option that best describes the client's current activity
- Fitness Goals: Describe what the client hopes to achieve
3. Body Measurements
- Height & Weight: Record in centimeters and kilograms
- Body Circumferences: Neck, chest, waist, and hip measurements
- Body Composition: BMI and body fat percentage (can be calculated or measured)
4. Fitness Tests
- Cardiovascular Metrics: Resting heart rate and blood pressure
- Strength Tests: Push-ups and sit-ups capacity
- Core Endurance: Plank time in seconds
- Flexibility: Measurement in centimeters (from sit-and-reach test)
- Advanced Metrics: VO2 Max and grip strength (if available)
5. Additional Notes
- Trainer's Observations: Record visual assessment, posture analysis, movement patterns
- Recommendations: Initial training suggestions based on assessment
6. Signatures
- Both client and trainer should sign after reviewing the completed form
How to Use
For Fitness Professionals:
- Fill out the form during the initial client assessment
- Use appropriate measurement tools for accurate data collection
- Explain each measurement to the client as you record it
- Review the completed form with the client
- Print a copy for your records and provide one to the client
For Clients:
- Provide accurate information about your health history
- Ask questions if you don't understand any terminology
- Review the completed assessment with your trainer
- Keep a copy for your records to track progress over time
Printing Instructions
- Click the "Print Form" button at the bottom of the page
- The printed version will remove unnecessary elements and optimize layout
- For best results, use standard letter-sized paper (8.5" × 11")
Tips for Accurate Assessment
- Perform measurements at the same time of day for consistency
- Use calibrated equipment for accurate results
- Record measurements immediately after taking them
- Note any special conditions that might affect results (illness, lack of sleep, etc.)
- Re-assess at regular intervals (typically every 4-8 weeks)
